I have a QNAP TS420 it got hit with the deadbolt malware this year. If I have another QNAP nas as a backup directly connected to the TS420 via USB or Ethernet and the TS 420 gets another attack can the malware also infect the backup NAS that is connected to the TS420 ?

Theoretically, hackers could attack your USB drive. But practically hackers normally prepare mass attacks, and they do not consider USB as a target. This would be a targeted attack situation.
I would not worry about it. People who had two volumes also did not get affected. Normally hackers attack only volume1 not 2.
Remember to us Admin account only for maintenance. For daily use create a limited user account.

the potential of malware really puts me off getting a NAS!
